Office hours
Office hours vary for each
physician in the Clinic depending on the nature of
the practice. The Business Office, Insurance and Radiology are open Monday
thru Thursday, 8 a.m. to 5 p.m. and Friday 8 a.m. 12 p.m. Lab Services in the Main Clinic Building are
available Monday thru Thursday 7:30 a.m. to 5:30 p.m. and on Fridays 7:30
a.m. to 12:00 p.m.

After hours/emergency care
Each physician of the Clinic is listed in the telephone book with an
after hours number to call. Brochures for parents entitled "When
Should I Call the Doctor?" are available in the Pediatrics area of
the Clinic.

Appointments and Referral Authorizations
In certain specialties, a referral from a Primary Care Physician is
required prior to being seen as a new patient. You will be informed of
that when you make your appointment. If you are unable to keep your
appointment, please call your physicians office and let them know. In
the event that the physician is called out for emergency care, we will
make every effort to notify you and reschedule. If your health plan
requires you to select and use a primary care physician and/or your health
insurer requires pre-authorization for certain medical procedures or
specialty services, you need to verify that any required authorization is
taken care of prior to your visit.

Telephone Calls
Your call to the Clinic will be answered first by a Switchboard
Operator who will put your call through to your physicians office or
other area of the Clinic. Occasionally, it is necessary for your
physicians office to take a message and get back to you. We appreciate
your patience during these times. Our phone system also has voice mail
capability for leaving messages. If your call is an extreme emergency,
identify it as such so it can be dealt with accordingly.

Registration
As a new patient, and annually for returning patients, you will be
asked to complete a Patient Information Form. You may also need to
complete Medical History Forms and Insurance Release Forms. We apologize
for the paperwork, but hope that it pays off for you in the form of
accurate and expeditious claim handling. Please bring your insurance card with
you to
every visit.

Handicap Accessibility
The Clinic is handicap accessible and has dedicated parking
including van access. Wheelchairs are available and assistance
can be obtained at the Front Desk of all the buildings. If you need
assistance in getting between the buildings or in accessing an
administrative service, ask at the reception desk.

Business Information
Although your insurance policy is a contract between you and the
insurance company and payment for care is the patient's responsibility, Mowery Clinic wants to be your partner in this
complex process. The Clinic's Insurance and Business Office
staff will work aggressively on your behalf to assure that your
insurer pays for all legitimate benefits.

Medicare
All Clinic physicians are participating providers with Medicare;
we will electronically file all claims for you. You are
responsible for paying the amounts applied to deductible and the 20%
co-insurance and for those services not covered by the Medicare
Program. If you have Plan 65 (Blue Shield) or one of the many
other Medicare Supplement plans, the secondary claims will also be
filed electronically for you. You will not receive a statement
unless there is a balance after Medicare has been processed.

Blue Shield/Blue Select
All Clinic physicians are contracting providers with Blue
Shield. The Pediatricians are Primary Care Providers with
Premier Blue. The Pediatricians, Internists and Cancer
physicians are Primary Care Providers with Blue Select. All
other Clinic physicians are referral specialists in this
program. We will electronically file all claims for you and
payment, if any, will be forwarded back to us. You will not
receive a statement unless there is a balance after Blue Shield has been
processed.

Other Commercial Insurance Carriers
We file claims electronically to almost all carriers. You
may want to check with our Insurance Department to assure that we
have correct and complete information necessary to file your
claim. In the event you desire to file your own claims, please
notify the Insurance Department. We are not able to file all
special cancer insurance claims.

Medicaid (Title XIX)
The Pediatricians are contracting Primary Care Physicians and the
Clinic's remaining physicians are specialists for the Medicaid
program. We file all Medicaid claims for you . You must
bring the current month's medical card with you to each visit.
If you are in the process of applying for a medical card, advise the
receptionist. Services not covered by Medicaid will be the
patient's responsibility.

Workers' Compensation and Accident Care
Please indicate when making an appointment and at registration that
you are here for care due to a workplace illness/accident or any
other type of accident, i.e. auto. Failure to do this may
result in your claim being filed to the wrong company and denied by
your regular health insurance carrier. The Clinic reserves the
right to contact your employer prior to care for a work-related
injury or illness.

Deductible, Co-Pays and Remaining Balances
You are responsible for meeting deductibles and for paying any
portion of your bill not paid by your insurance carrier. If
your insurance contract calls for a Co-Pay, it is due at the time of
your visit. Many insurance companies require
pre-authorization, second opinion and limited/pre-approved hospital
stays. We will be glad to help you meet these
requirements. However, we can not assume responsibility for
your bill if they are not met. Please inform the nurse if your
coverage has any of these limitations.

Payment and Collection Policy
Other clinics throughout the United States typically expect
payment for all in-office provided medical services to be made at
the time services are given. Mowery Clinic (except for
insurance required time of service office co-pays) usually allows
payment to be deferred until you receive your first statement.
Payment for the full amount of your account balance is due and
payable upon receipt of your statement. We realize that a
person's current financial situation may sometimes not allow for
their entire balance to be paid at one time. In such circumstances,
appropriate alternative payment arrangements may be made by
contacting the staff in our Business Office. Accounts will be
considered delinquent if not paid within thirty (30) days of a statement
or if any arranged payment terms with our Business Office staff are
not honored. Interest, at the rate of ten percent (10%) per annum,
will be charged on all account balances that are over 120 days old.
The Clinic accepts Discover, VISA and MasterCard. The Clinic
retains the right to request payment at the time care is provided
for certain elective services and when an account is delinquent or
has previously been in a non-current status. Delinquent
accounts may be referred to an outside agency for collection.

Medical Records
The medical record is the property of Mowery Clinic,
L.L.C. and shall be maintained to serve you and your health care
providers. However, the information contained in the record
belongs to the patient, and the patient may to a large degree
control the access to this information. If you are
transferring to the Clinic, please request that we send for your
medical records. Your records are confidential and copies will
not be released to anyone without your written consent except as
allowed by Kansas Statute.
